Exit Interview Questions

I'm hoping someone can help me with some exit interview questions that I should be prepared to ask an employee who is leaving. At the moment, I'm struggling to come up with questions from here that would make the most sense to ask and will help me obtain an honest response. Does anyone on the forum have some suggestions that they can give me?
